Proforma AA has to be filed for Chief functionary/Board members while submitting form FC 3A (Registration), FC 3B (Prior Permission), FC 3C (Renewal of registration) and for addition of any such members during the term of FCRA registration in Form FC 6E. The Proforma AA has been revised. Please use the Proforma sample attached for reference.
